Accessing someone else’s information (Proxy access)
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

Pride in Practice
Friarwood Surgery is supported by LGBT Foundation’s Pride in Practice service to improve the experiences of LGBTQ+ people accessing primary care services.
We are open to all and are dedicated to protecting diversity and equality.
We are here to listen to suggestions and feedback from our LGBTQ+ patients on their experiences with us.
If you have something you want to share or feel would help us in this process, please let us know.
It is important to us if you feel you feel able to share your sexual orientation, gender identity and trans status with us so that we may be able to offer the most appropriate support to you within our practice and refer you on to any relevant services if you may need them.

Transgender Patients
Updating Your Details
We are committed to providing a respectful, inclusive, and supportive service for all our patients. Transgender and non‑binary patients are welcome to update their personal details with us at any time.
Updating Your Name and Gender Marker
You can request a change of name or gender marker on your GP record without needing:
- A Gender Identity Clinic appointment
- Hormone treatment
- Surgery
- A Gender Recognition Certificate
Name changes:
UK law allows anyone to use any name they are known by. However, Primary Care Support England (PCSE) recommends that GP practices see a deed poll before updating a patient’s name on their medical record.
For this reason, we will ask for a deed poll when changing your name.

Gender marker changes:
Changing your name does not automatically change your gender marker. If you would like your gender marker updated, please let us know and we will request this through PCSE on your behalf.
What Happens When Your Gender Marker Is Changed?
- You will be issued a new NHS number (this cannot be reversed).
- You will be registered at the practice as a new patient.
- Your previous medical history will be transferred to your new record.
- Information that could reveal your previous gender identity will not appear in the new record.
- Important details will be kept using gender‑neutral language where needed (e.g., “the patient had a smear on…”).
This helps protect your privacy and ensures we comply with the Gender Recognition Act 2004, which restricts disclosure of a person’s trans history.

Screening Invitations
Changing your gender marker may affect which routine NHS screening invitations you receive. Screening programmes are based on the gender recorded on your NHS record, not on the organs you have.
Trans women and non-binary people assigned male at birth who are registered with a GP as female:
- are invited for breast screening
- are invited for bowel cancer screening
- do not need cervical screening as they do not have a cervix
- are not routinely invited for abdominal aortic aneurysm (AAA) screening, but can request screening
Trans women and non-binary people assigned male at birth who are registered with a GP as male:
- are not routinely invited for breast screening, but can request screening
- are invited for bowel cancer screening
- do not need cervical screening as they do not have a cervix
- are invited for abdominal aortic aneurysm (AAA) screening
Trans men and non-binary people assigned female at birth who are registered with a GP as female:
- are invited for breast screening
- are invited for bowel cancer screening
- are invited for cervical screening
- are not invited for abdominal aortic aneurysm (AAA) screening
Trans men and non-binary people assigned female at birth who are registered with a GP as male:
- are not routinely invited for breast screening, but can request screening
- are invited for bowel cancer screening
- are not routinely invited for cervical screening, but can request screening
- are invited for abdominal aortic aneurysm (AAA) screening, but do not have a high risk of AAA

If You Are Already Under Specialist Care
If you are currently being seen by a Gender Identity Clinic or any other specialist service, we will inform them of your updated details and new NHS number to ensure your care continues smoothly.
