Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
- Medicals for pre-employment and driving requirements (HGV, PSV etc.)
- Insurance claim forms
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
We are only able to provide the above service to patients registered with Friarwood Surgery.
Please allow a minimum of 28 days for these forms to be completed by the surgery.
Payment is required by the practice prior to the forms being completed.
Proof of address / Witness Signatures
We are unable to provide proof of address for residency or witness signatures of any kind i.e. passports, pensions, power of attorney etc
Extreme Sports
We do not sign forms for extreme sports such as sky diving, parachuting, mountain climbing, scuba diving, marathons, swimming contests. These need to be arranged with a private specialist.
