COVID-19 vaccinations for 5-11s

 The NHS is now offering COVID-19 vaccinations to all children aged 5-11, in line with the latest advice from the Joint Committee on Vaccination and Immunisation (JCVI). As well as protecting children against serious COVID infection and Long COVID, this will reduce the risk of children passing on the infection to others.

 

The practice is currently not offering vaccinations for this age group, unless they are within one of the at-risk groups who will be invited, but you can book an appointment at a convenient community or pharmacy vaccination centre. Please book online at www.nhs.uk/covidvaccine or by calling 119, free of charge.

Take 20 minutes out of your day (or evening or weekend) to finally catch up on your health. These vital health check ups can spot your risk of diabetes, heart & kidney disease, stroke and dementia.

Registration with the practice

PreRegistrationWhen you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

Due to the coronavirus pandemic we are having to work differently.

Please print a registration form and appropriate questionnaire per person. You will also need to provide a copy of the relevant  photo ID (passport, drivers licence etc.) and a utility bill for proof of address to validate your registration. Children require a copy of their birth certificates showing parents names. 

Please ensure you add your 10 digit NHS number to the registration form, without it we cannot proceed with your registration. 

The forms can either be posted to the surgery or left in the postbox outside the building.
